To register with Talk to a Doctor.co.uk Locums we need to see the following documents from you:

CV (With 2 referees) GMC Certificate Hepatitis B&C Report Passport or work visa (CRB) Criminal Record Bureau

 

For GPs we require all the above documents and additionally:

PCT Medical Performers List Letter MDU/MPS Medical Indemnity PMETB/JCPTGB GP Certificate
